About melasma treatment

Melasma is a darkening of the skin from pigment lying deep in the skin. One Bangkok hospital describes it as large dark or brown patches on the forehead, cheeks, nose and above the lip, often linked to hormones, such as pregnancy or the contraceptive pill.

Bangkok hospitals and clinics treat it with lasers, light, and injections of tranexamic acid, usually as a course of sessions.

Techniques and approaches

The same procedure name covers different operations. These are the approaches a clinic will offer, and what separates them.

Q-switched laser

Laser toning sold by two hospitals for melasma, freckles and dark spots, by the session or as a course of five.

Pico laser

Picosecond lasers such as PicoWay and Discovery Pico, sold by one hospital and one clinic, often in packages that also cover acne scars and freckles.

Injection

One hospital sells a melasma injection of 5 ml of tranexamic acid (Transamin).

Light and other lasers

One clinic sells a light treatment (VIPL) and a Z-Smooth laser for melasma, freckles and dark marks.

The decisions you will make

These come up at consultation. Knowing them beforehand makes the conversation shorter and the plan clearer.

Laser or injection

Ask the doctor which suits your melasma, and whether they advise combining them.

A course

One clinic says melasma usually needs about three to five sessions or more. Ask for the course price and how far apart the sessions are.

Who should wait

One hospital says its Q-switched laser is not suitable with inflamed acne. Another asks you to tell the doctor about any medicine that makes the skin sensitive to light.

About 1 day

What the trip involves

Clinics vary, and your clinician’s plan overrides anything on this page. The shape is consistent.

  1. 1Before you fly

Melasma is treated over a course of sessions, so one trip covers only part of it. One hospital advises avoiding the sun for one to two weeks before.

  2. 2Assessment

A doctor examines the skin and decides the method and the number of sessions.

  3. 3The session

One clinic says it usually applies numbing cream before pico laser.

  4. 4Afterwards

Slight redness, which one clinic says improves over one to two days.

  5. 5The first weeks

One hospital advises sunscreen and no strong sun for one to two weeks, no scrubs or acids until the doctor allows, and no sauna or hard exercise for two to three days.

  6. 6Back home

Keep using sunscreen. Ask the doctor what to do to keep the melasma from coming back.

Ask these before you pay a deposit

Formatted as a numbered list, ready to email a clinic.
  1. Who is treating me, and what are their qualifications?

A clinic name is not a clinician. Ask for the individual, their registration, and how often they do this procedure.

  2. What is included in the price, and what is not?

Facility fees, anaesthesia, materials, medication, follow-up appointments and take-home items are often quoted separately.

  3. Is the deposit refundable, and under what conditions?

Including if the clinic reschedules, or if the in-person assessment changes the plan.

  4. What if it needs correcting?

Ask for the policy in writing: who pays, within what window, and whether it covers a return trip.

  5. Who do I contact after I go home, and in what language?

A name, a channel, and an expected response time. Aftercare that ends at the airport is not aftercare.
